Online Credit Application
Thank you for your interest in Mid-West Fertilizer. In order to do business with Mid-West Fertilizer, an application form must be completed and returned to our office.
To complete a credit application online and sign the document electronically, an email address is required. If an email is not available, a paper application can be downloaded, completed and mailed to our office.

Oklahoma residents must send a copy of sales tax exemption permit to mwfcredit@mwfert.com. Without proof of exemption on file, all Oklahoma sales will be considered taxable.
